2. Appointments

• Scheduling: Appointments can be scheduled via phone at 410-771-8200, or in person.
• Cancellations: We kindly request a 24-hour notice for any cancellations or rescheduling of appointments to avoid a cancellation fee of $25.
• Late Arrivals: If you arrive late for your child’s appointment, we may need to reschedule or shorten the appointment to accommodate other patients.
  
  

3. Payment and Insurance

• Payment: Payment for services is due at the time of service unless prior arrangements are made. We accept cash, credit cards, and other payment methods.
• Insurance: We accept most major insurance plans. However, you are responsible for confirming coverage and understanding your benefits. Any charges not covered by insurance will be your responsibility.
• Financial Assistance: If you are unable to pay the full amount at the time of service, please inquire about payment options.
   

4. Patient Responsibilities

• Accurate Information: You agree to provide accurate, current, and complete information regarding your child’s medical history, dental condition, and insurance coverage.
• Follow-Up Care: It is your responsibility to follow the care instructions provided by our dental professionals and attend any necessary follow-up appointments.

We are committed to protecting your privacy. All patient information is stored securely and used solely for purposes related to your child’s dental care and treatment. We comply with all applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

6. Treatment Consent

• Informed Consent: By scheduling an appointment, you give consent for the dental team to assess your child’s oral health and recommend appropriate treatments.
• Risks and Benefits: Before any procedure, our team will inform you of the risks, benefits, and alternatives to treatment for your child. You have the right to ask questions and make an informed decision regarding your child’s care.
  
  

7. Promotional Information Consent

By using Dentistry for Kids’ services, including signing up for text message communications, you expressly consent to collecting, using, and sharing your child’s personal information as outlined in our Privacy Policy.

You further consent to receive text messages from Dentistry for Kids including transactional and promotional messages.

7a. Message Frequency and Content

By subscribing to Dentistry for Kids text messaging service, you agree to receive promotional, transactional, or informational messages related to our services, updates and special offers. You can expect to receive up to 10 messages per week. Message and data rates may apply.

7b. Opt-out Procedures

You may choose to stop receiving promotional text messages from Dentistry for Kids at any time. To opt out, simply reply to any text message you receive from us with the word ‘STOP’. Once we receive your opt-out request, we will promptly remove your number from our promotional messaging list. Please note that opting out of promotional messages will not affect your ability to receive important service-related communications.

7c. Liability and Disclaimers

Dentistry for Kids is not responsible for any delays, failures in delivery, or any other issues related to the transmission or receipt of text messages. Delivery of text messages is subject to effective transmission by your mobile carrier and is not guaranteed by Dentistry for Kids.

By subscribing to our SMS services, you acknowledge and agree that Dentistry for Kids will not be liable for any damages, losses, or injuries arising from or related to the use or failure to receive any text messages, including but not limited to, delays, non-delivery, or technical issues. Your use of our SMS services is at your own risk, and we provide our services on an ‘as-is’ basis without any warranties of any kind, express or implied.

Dentistry for Kids is committed to protecting the security of your child’s personal information. We implement industry-standard security measures to safeguard your data against unauthorized access, use, or disclosure. However, it is also your responsibility to protect the confidentiality of your child’s account information, and any passwords associated with your use of our services.

You agree to notify Dentistry for Kids immediately of any unauthorized use of your account or any other security breach. Dentistry for Kids will not be liable for any loss or damage arising from your failure to protect your child’s account or personal information adequately. By using our services, you acknowledge and accept that no data transmission over the Internet or mobile networks can be guaranteed to be 100% secure, and therefore, you use our services at your own risk.

In some cases, we may refer your child to third-party specialists or providers for services not available at our clinic. We are not responsible for the services provided by those third parties, but we will ensure that any referrals are made with your best interests in mind.
